Why is sustainability important?
Sustaining the Community Health Worker (CHW) workforce is vital for the well-being of CHWs, the communities they serve, and the organizations that employ them. CHWs are trusted members of the communities they serve and/or have a unique understanding of the community’s needs. CHWs often encounter the same challenges as the people they work with. These shared lived experiences enable them to provide culturally competent support, play vital community roles, and effectively bridge gaps between healthcare systems and communities. envision has gathered resources and developed new tools to support the development and implementation of CHW sustainability plans.

CHW financial sustainability toolkit (FST)
The CHW Financial Sustainability Planning Toolkit, created by envision in collaboration with national experts, is a comprehensive resource designed to help CHWs and CHW allies achieve financial sustainability.
While initially developed for recipients funded under the Centers for Disease Control and Prevention (CDC) initiative CHWs for COVID Response and Resilient Communities (CCR), it has since been adapted to support all CHWs and CHW allies, regardless of their current funding situation.
The Toolkit addresses the complex challenges of CHW workforce sustainability and emphasizes the importance of relationships and relationship-building as a key strategy for advancing financial sustainability goals.
