Why is sustainability important?

Sustaining the Community Health Worker (CHW) workforce is vital for the well-being of CHWs, the communities they serve, and the organizations that employ them. CHWs are trusted members of the communities they serve and/or have a unique understanding of the community’s needs. CHWs often encounter the same challenges as the people they work with. These shared lived experiences enable them to provide culturally competent support, play vital community roles, and effectively bridge gaps between healthcare systems and communities. envision has gathered resources and developed new tools to support the development and implementation of CHW sustainability plans.

Why are CHWs important?

Community Health Workers (CHWs) are essential because they build trust, bridge gaps between communities and health systems, address social barriers to health, improve access to care, and promote health equity by providing culturally relevant education and navigation for individuals and families, leading to better health outcomes and reduced costs. They act as trusted, frontline links, helping people navigate complex systems, manage chronic conditions, and connect with crucial social services.
