What is the State Learning Collaborative?

This collaborative brings together up to 15 state health departments focused on strengthening the policies, structures, relationships, and leadership conditions needed to grow and sustain CHW programs/support in state health departments. Through shared learning, participants will co-develop, refine, and utilize a practical checklist to guide state health departments in advancing CHW programs in ways that align with Health department priorities while honoring CHW values, leadership, and long-term workforce sustainability goals.

This learning collaborative will address topics like:

  • State endorsed CHW definition and training standards 

  • CHW credentialing and credentialing decision making 

  • CHW workforce taskforce development

  • Creation of CHW named positions and/or office of CHWs

The key to making participation truly impactful is engaging CHW leaders in your state and bringing their feedback into the collaborative. Their insights will help shape discussions and the tools we create, ensuring resources are grounded in real-world experience and aligned with both CHW workforce priorities and your department’s goals.



Objectives of the Collaborative

By the end of the SLC, participants will be able to:

  • Identify key policies, leadership structures, and conditions that support sustainable, CHW programs in State Health Departments

  • Understand where their department stands in advancing CHW programs aligned with CHW values and roles

  • Strengthen collaboration and shared accountability between state health departments and CHW partners.  

  • Foster meaningful peer relationships and shared learning with other state health departments to exchange ideas and advance CHW programs together.

  • Co-develop a practical, CHW-informed checklist to guide the growth and sustainability of CHW programs
