We invite you to join our cohort based, interactive learning community that will support you to lead your organization through the “Sustainability from the Inside Out” Self-Assessment Toolkit. In teams of two (including a minimum of 1 CHW representative) you will:

  • Learn the different components of the toolkit

  • Be supported to conduct the toolkit activities in real time at your organization

  • Receive peer and subject matter expert technical assistance to complete the toolkit

  • Better understand your organization's readiness to support thriving CHW programs 

  • Help refine the toolkit for wider dissemination in 2027.

This is a highly interactive and experiential learning community, and requires participants to remain engaged (camera on) during each session. Consider your own capacity to be present and lead your organization through the toolkit components (with support from the learning collaborative facilitators and partners) before applying to participate.  

As a reminder, participating organizations MUST apply in teams of two people, with at least one person identifying as a Community Health Worker (CHW), and one person (either the CHW or the other team member) serving as the Project Coordinator.
