The Wisconsin Department of Health Services (WI DHS) is a recipient of the National Partnership cooperative agreement from the CDC. WI DHS partners with national CHW organizations, including envision, to offer technical assistance and connections to peer support to state, Tribal, local, and territorial health departments to integrate CHWs into public health and clinical settings, address non-medical health needs, and sustain CHW programs.
